Non-UK Patient Registration
If you are not currently registered with a doctor in the UK you need to fill in a more detailed application form. This form is available in English and in a range of other languages. Please click on the respective links below to download the form in English and/or in translation. Bring this form with you to reception where you will also be asked to complete a new patient health questionnaire.
