New Patient Registration

Registrations are welcome from all patients living within our Practice Area.

There are two easy steps you must follow which will authorise us to obtain your medical records and enable us to take over your care safely. 

Step 1 - Leave in your signed medical card.  Don't worry if you don't have a medical card.  You can complete a Health Service registration form which is available at reception.

Step 2 - Complete a new patient health questionnaire which is available at reception. 

That's it!  We do the rest.  You are now welcome to use our services as you need us.

 

In general, if you are taking three or more repeat drugs (long term drugs) we will ask you to book an appointment with one of our doctors.  This appointment is nothing to worry about.  The doctor will assess your condition, review your medications and explain how we will manage your care in the future.
